Many OMT providers issue superbills but do not submit medical claims directly. We offer a secure, streamlined solution to submit your claim on your behalf.

For a flat fee of $40, we will file your medical claim and provide tracking information for your records.

How the Process Works

Complete the $40 claim filing payment.

Step 3: Submit Account Information

After payment, you will complete our Submit Account Information Form, where you will be prompted to provide:

  • Patient name
  • Date of birth
  • Email address
  • Phone number
  • Patient or guarantor name
  • Employer information
  • Treating provider name or company name
  • Upload the front and back of your medical insurance card

Click Submit once complete.

What Happens Next: Once your information is received

  • Our team is notified immediately
  • We will contact you to obtain your superbill and any additional required documentation
  • We will submit your claim to your medical insurance carrier
  • You will receive claim tracking information (trace number)
  • We will communicate any updates as needed!

Your OMT provider will also have access to a shared tracking grid confirming submission status.
